Why It Matters Where You Get This Test

This isn’t a wellness amenity. It’s cardiology.

At a longevity center or fitness clinic

✕

Test is a standalone product — a number you leave with

✕

Results interpreted by a fitness coach or wellness staff, not a physician

✕

No integration with your cardiovascular risk profile, medications, or labs

✕

No cardiac monitoring during the active stress test

✕

Data sits in an app — no clinical follow-through

At Di Pietro Health

The Process

How the PNOE Breath Test Works

Indirect calorimetry is the gold standard for measuring metabolism. You breathe into a precision mask connected to the PNOE analyzer — it measures oxygen you consume and carbon dioxide you produce, breath by breath, in real time.

01

Arrive & Prepare

For resting tests, fast for 4–6 hours and avoid caffeine beforehand. For VO₂ Max testing, come in comfortable workout clothes prepared for a graded exercise stress test. Our team will walk you through everything.

02

Put on the Mask

A medical-grade silicone mask forms a precise seal over your nose and mouth, connected to the PNOE analyzer. For resting tests, you simply lie back and breathe normally for approximately 10 minutes.

03

The Test

The PNOE captures breath-by-breath oxygen and CO₂ data. The active VO₂ Max test pairs the breath analyzer with a cardiac stress test — progressive exercise on a treadmill or bike with continuous heart monitoring — until maximum effort is reached.

04

Your Results

Your 23-biomarker report is reviewed and interpreted by Dr. Di Pietro, who translates the data into a personalized nutrition, exercise, and cardiometabolic strategy tailored to your goals.

Two Tests. Complete Metabolic Picture.

Please note: PNOE metabolic testing is a standalone diagnostic service and is separate from a preventive cardiology consultation or office visit. If you are interested in a comprehensive cardiology evaluation with Dr. Di Pietro, please inquire about scheduling a consultation in addition to your metabolic test.

Resting Test

Resting Metabolic Rate (RMR)

$250

Most calorie estimates are guesses — built on population formulas that can be off by 20% or more. The PNOE measures exactly how many calories your body burns at rest, and what fuel it’s using to do it. It’s the difference between a plan built on averages and one built on your actual physiology.

✓

Your true caloric baselineBreath analysis measures exactly how many calories your body burns at rest — replacing formulas that can be off by 20% or more with your actual metabolic rate.

✓

Fat vs. carbohydrate utilizationYour Respiratory Exchange Ratio (RER) reveals how efficiently your body burns each fuel source — identifying metabolic inflexibility, insulin resistance patterns, and why weight loss may have stalled.

✓

Precision nutrition strategyResults directly inform your caloric targets and macronutrient ratios — whether your goal is sustainable fat loss, GLP-1 optimization, metabolic recovery, or long-term cardiometabolic health.

Active Test

VO₂ Max & Cardiometabolic Fitness

$350

VO₂ Max is the most powerful independent predictor of cardiovascular mortality — stronger than blood pressure, cholesterol, or smoking status. Knowing yours isn’t just for athletes. It’s essential preventive cardiology.

Includes a cardiac stress test — continuous ECG heart monitoring throughout graded exercise, performed under physician supervision.

✓

Your cardiovascular longevity scoreWe measure your VO₂ Max and ventilatory thresholds (VT1/VT2) — biomarkers among the strongest independent predictors of all-cause and cardiovascular mortality — with clinical-grade precision.

✓

Personalized heart rate training zonesRather than generic estimates, your ventilatory thresholds define your exact Zone 2 ceiling and anaerobic boundary — so every workout targets the cardiovascular adaptations that actually extend your healthspan.

✓

23-biomarker metabolic snapshotOxygen chain efficiency, biological age, breathing mechanics, and fat oxidation capacity — assessed in a single non-invasive session with a physician-reviewed report and personalized action plan.

What We Measure

Key Biomarkers Your Report Includes

The PNOE analyzes how oxygen flows through your lungs, heart, and cells — producing actionable data on each system’s contribution to your metabolic health and longevity.

VO₂ Max

Aerobic Capacity

The maximum rate at which your body can use oxygen during exercise. The single strongest predictor of cardiovascular mortality and a key marker of biological age.

Fat-Max

Peak Fat Oxidation

The exercise intensity at which your body burns fat at its highest rate — a critical target for weight loss, metabolic flexibility, and endurance performance.

RMR

Resting Metabolic Rate

The exact number of calories your body burns at rest. Essential for building nutrition plans that actually match your physiology, not population averages.

VT1 / VT2

Ventilatory Thresholds

The intensity at which your breathing pattern shifts — defining your true Zone 2 ceiling and anaerobic threshold for precision training zone prescription.

RER

Respiratory Exchange Ratio

The ratio of CO₂ produced to O₂ consumed, revealing whether your body is burning fat or carbohydrates — and how metabolically flexible you are.

Bio Age

Metabolic Biological Age

A composite assessment of your metabolic and cardiovascular function compared to age and sex norms — an objective benchmark for tracking longevity interventions over time.

Ideal Candidates

Who Benefits from Metabolic Testing?

Weight Loss & GLP-1 Patients

If you’re on semaglutide or tirzepatide, or struggling with a stalled metabolism, RMR testing reveals whether adaptive thermogenesis has slowed your burn — and how to correct it without sacrificing muscle.

Endurance Athletes & Performers

Runners, cyclists, and triathletes receive precision heart rate zone prescriptions derived from their actual ventilatory thresholds — not wrist-based estimates — enabling smarter, more effective training.

Longevity & Preventive Cardiology

VO₂ Max is among the most powerful predictors of how long — and how well — you’ll live. Annual testing establishes your baseline, tracks improvement, and validates that your prevention program is working.

Hormone & Metabolic Health Patients

Thyroid dysfunction, hormonal shifts, and aging can profoundly alter metabolic rate and fat utilization. PNOE testing quantifies exactly what’s changed — providing objective data to guide hormonal optimization and nutrition.

“A VO₂ Max number from a gym is interesting. A VO₂ Max number interpreted by a cardiologist — in the context of your lipids, your coronary calcium score, your blood pressure, your family history — is a clinical tool. That’s the difference between a data point and a plan.”

FREQUENTLY ASKED QUESTIONS

No. PNOE metabolic testing is available as a standalone service. However, because the active VO₂ Max test includes a cardiac stress test, results are reviewed and interpreted by Dr. Di Pietro — giving you the clinical context no fitness studio can provide.
The RMR (Resting Metabolic Rate) test measures how many calories you burn at rest and what fuel your body uses — critical for nutrition planning and weight management. The VO₂ Max test is an active test performed during graded exercise that measures your cardiovascular fitness, aerobic capacity, and training zones — and includes a cardiac stress test with continuous ECG monitoring.
For the RMR test, fast for 4–6 hours and avoid caffeine beforehand. For the VO₂ Max test, wear comfortable workout clothes and avoid strenuous exercise the day before. Our team will provide specific preparation instructions when you book.
Metabolic testing is a cash-pay service. RMR testing is $250, VO₂ Max testing is $350, and the Complete Metabolic Profile (both tests) is $499. You may be able to use an FSA account if you have one.
The resting RMR test takes approximately 10 minutes. The active VO₂ Max test (including the cardiac stress component) typically takes 45–60 minutes in total including preparation and cool-down.
Yes — and we encourage it. The metabolic test results are most powerful when integrated with a comprehensive preventive cardiology evaluation that includes your lipid profile, imaging, and full risk assessment. Please inquire when booking if you would like to combine both.
